DH and I will be moving in the next few months, when we do I will be starting a Women's Ministry. I am looking for questions to ask th eladies to get to know them, find out what their interests are. So what kind of questions would you suggest?

Following are some very basic questions that I asked on mine. I have found that it is best to keep it simple otherwise they might be overwhelmed and less likely to respond. Another thing is that you might have to stay on top of them to get the responses back. I made a list of every lady and marked them off as I got them back. Those that I did not get back I "gently persuaded" until they finally filled them out. 
My favorite part of the survey is the salvation testimony question. When one of our elderly ladies passed away I was so glad I had her testimony in writing. I shared it with our ladies and we all found great comfort in it after her passing.
I will be happy to pass this along in Word Format via email. Then you can tweak it to suit your needs/questions.

Julia has a form that you could use too. Maybe you could pick what you want from the suggestions given and make up one that is applicable to your WM.
